Investigation Summary
At approximately 11:50 a.m. on August 1, 1997, Employee #1, a crane operator, was told to go to the cab and to start booming up. He began the process and then suddenly stopped. Coworkers found Employee #1 slouched over in the operator's cab, having suffered a massive heart attack. He was transported to the hospital, where he was pronounced dead. Employee #1's death was attributed to an acute myocardial infarction due to chronic hypertension.
