Event: 09/19/1998
Employee killed when caught in portable tar mixer

At approximately 9:00 a.m. on September 19, 1998, Employee #1 began cleaning a portable tar mixer that had been dropped off at his home two days earlier. Apparently, Employee #1 was standing on a concrete block cleaning the tar mixer when for some reason he started the machine. His left arm became caught and was pulled into the mixer, followed by most of his upper body and midsection. At approximately 12:15 p.m., Employee #1 was found with both legs still on the concrete block and the mixer running. He was pronounced dead at the scene due to compressional asphyxia of the chest. There were no witnesses to the accident.
