Employee killed in fall from metal warehouse roof

Employee #1 was assigned to clean caked grain dust off the metal roof of a warehouse at a grain pellet plant. The vent for the plant is situated over the warehouse roof, and grain dust cakes up on the roof and must be cleaned off periodically. Employee #1 asked a coworker to help him and then climbed up a fixed ladder to access a catwalk that overlooked the warehouse roof. It had been raining and the wet grain dust made the metal roof surface slippery. Employee #1 fell 18 ft off the wet roof onto the concrete pavement, and was killed. Informal practice at the site called for Employee #1 to toss a rope to the helper on the ground. The coworker would then tie the rope to a hose, enabling Employee #1 to pull up the hose to the roof and wash off the caked dust. Apparently, this procedure was not followed on the day of the accident. Employee #1 had not taken a rope with him to the catwalk, and the coworker was trying to toss the rope up to Employee #1 when he fell. Neither the rope nor the hose were found with Employee #1.
