Employee killed in fall while installing window

Employee #1 was installing a new window as part of some light residential construction repairs. The homeowner heard a noise and found Employee #1 on his back, suffering from head injuries. He was killed. There were no witnesses to this accident. Although a ladder was in place, it is not clear whether Employee #1 fell from the ladder. The position of his body indicated that he either fell out of the window or slipped while standing on the deck.
