Investigation Summary
At 1:55 p.m., Employee #1 and three coworkers were working from a steep 12:12 pitch roof with a ground-to-eave height of 20 ft 7 in. Slide guards were partially set up on the south-facing side just above the gutter, but they did not extend to where Employee #1 was straddling the ridge. He was attempting to install a sheet of cut plywood on the east-facing side and had a hammer in his right hand while holding onto a piece of furring strip with his left hand. The furring strip broke away and Employee #1 fell to his left and over the ridge to the cement sidewalk. He was killed.
