Investigation Summary
On December 10, 1999, Employee #1 was using a drop-down flatbed trailer to deliver tires to a distributor. The load included two 8 ft 9 in. diameter, 2,500 lb tires of the type used for quarry trucks. The two large tires were standing on their sides, and two smaller tires had been stacked horizontally behind them for support. The supplier had secured the large tires using a chain strap over their tops and two tightened lanyards passed through them. After entering the yard of the distributor, Employee #1 secured the help of a forklift operator and a laborer to assist him. He then began to remove the two lanyard straps holding the large tires to the flatbed while the forklift operator maneuvered into place to hold the large tires. The forks of the lift truck were still not touching the tires when the laborer noticed that Employee #1 was releasing the second lanyard that went through the tires. Before Employee #1 gave the signal to unload the tires, one of them fell off the flatbed and crushed him. He was administered first aid but he died as a result of his injuries.
