Investigation Summary

Employee #1 was part of a three-man crew of housing custodians moving a 980-pound printing press from a dormitory to the recycling center. They were using a cargo van with a hydraulic tailgate. They arrived at the recycling center, and all three moved the printing press to the rear door of truck. One employee went to the recycling office, while the other two manhandled the press off the back of the truck into a metal recycling bin. While pushing the press, Employee #1 lost his footing, fell 12 ft into the bin, and the press toppled in onto him. Employee #1 received crushing injuries to head and chest. Employee #1 was taken to the hospital by air ambulance and died two days later.
