Investigation Summary
At approximately 7:50 a.m. on the day of the accident, Employee #1 attended the daily work meeting to assign tasks at the site of a courthouse under construction. He was scheduled to leave town later that morning, but at the end of the meeting, the foreman asked him to bend some metal roofing valleys before departing. The valleys were on the west side of the building. All the other workers were on the north side of the roof when Employee #1 fell approximately 50 ft to the ground. He was killed. There was dew on the roof where Employee #1 was working, and he was not using any form of fall protection.
