ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS
An employee was troubleshooting a grinder. He went to the disconnect switch and removed the cover on the cabinet, allegedly to check for voltage. He apparently contacted energized parts inside the cabinet and was electrocuted.
