Investigation Summary

Investigation Nr: 200000347
Event: 09/26/1997
Employee killed when struck by steel column

Employee #1, one of five structural steel workers at a construction site, was at the east end of a 24 ft steel I-beam, attempting to connect it to a 21 ft high steel column. He was on a ladder, and his head was approximately 10 ft above the cement slab on which the ladder was placed. The column became displaced from the foundation bolts during the connecting process and struck Employee #1 on the upper right rear side of the head. He was knocked from the ladder and landed, face-first, on the cement slab. The medical examiner determined that Employee #1 died from being struck by the column, bit by the subsequent fall.
