STRUCK ON HEAD BY LOG; NECK PUNCTURED ON LIMB STUB

THE KNUCKLE BOOM OPERATOR HAD JUST PLACED TWO WET AND MUDDY YELLOW PINE POLES ONTO THE REAR RICK OF PULPWOOD POLES. HE WAS SWINGING THE BOOM TO PICK UP AND LOAD THE LAST TWO POLES. EMPLOYEE #1 PICKED UP THE TIE-DOWN CHAIN AND RAN BETWEEN THE KNUCKLE BOOM AND THE LOG TRAILER. IT IS NOT KNOWN WHY EMPLOYEE #1 TOOK THIS ACTION, SINCE IT IS AGAINST ALL PREVIOUS INSTRUCTIONS. AN ALREADY LOADED POLE SLID OFF THE STACK AND STRUCK EMPLOYEE #1 IN THE BACK OF THE HEAD. HE WAS PROPELLED ONTO A PILE OF CEDAR POLES, WHERE HIS NECK WAS PUNCTURED BY A PROTRUDING LIMB STUB. HE WAS KILLED.
