CONSTRUCTION EMPLOYEE INJURED IN FALL WHEN LADDER SLIPPED

EMPLOYEE #1 WAS CLIMBING AN ALUMINUM EXTENSION LADDER TO DRILL HOLES IN A MASONRY WALL WITH A HAMMER DRILL. THE LADDER, IN ADDITION TO HAVING A DEFECTIVE SAFETY FOOT, WAS PLACED WITH THE LADDER BASE TOO FAR FROM THE WALL (TOO GREAT AN ANGLE FROM VERTICAL). AS EMPLOYEE #1 ATTEMPTED TO CLIMB THE LADDER, THE BASE SLIPPED OUT FROM UNDER IT, CAUSING EMPLOYEE TO FALL 6 1/2 FEET TO THE CONCRETE SURFACE. HE SUFFERED A CUT LIP.
