Investigation Summary
At approximately 11:00 a.m. on May 5, 2011, Employee #1 was performing a delivery for C.O. Delivery, a sub-contractor for FedEx Ground, at a local Macy's distribution facility. He arrived at approximately 10:30 a.m. and backed his vehicle, a 28 foot 2000 International 4700/T444E connected cab delivery truck, into bay 37. There were trailers parked in both adjacent bays. At this point, events are not clear, as there were no actual witnesses to the event itself. The employee exited the truck and was performing some task between his truck and the Knight Transportation trailer parked in an adjacent bay. As he was performing this task, the vehicle rolled and pinned him, standing upright, between his vehicle and the Knight trailer. He was in that position for an unknown period of time, but it may have been as long as 30 minutes. While in this position, Employee #1 was killed from crushing injury to his chest.
