EMPLOYEE INJURED FROM A FALL ON A NOT SECURED LADDER

On May 21, 1985, several employees were using a portable metal 40 ft ladder to gain access to the ceiling area of the 3rd floor of the structure. After completing the work, an employee moved the ladder to the side to clear the area. The position of the ladder was upright from the 3rd floor extending up to the roof approximately 4 ft from the 3rd floor's perimeter edge and left unsecured. Employee #1 was unaware that the ladder was not secure and ascended the ladder to reach the roof area of the 3rd floor. As he traveled towards the top of the ladder, the ladder became unstable displacing from its original location, and this action caused him to fall from the roof to the floor. He was injured by the fall of 15 ft.
