STRUCK BY ACCESS DOOR TO PERSONNEL HOIST

ON JULY 9, 1985, AT APPROXIMATELY 2 PM, EMPLOYEES #1 AND #2, BOTH EMPLOYED BY THE AMPAT SOUTHERN CORP., WERE STANDING AT GROUND LEVEL BY THE WEST FACE OF THE BUILDING. THIS IS THE LOCATION OF THE TWO PERSONNEL HOISTS. THE SOUTH PERSONNEL HOIST, #WH 2060, WAS DESCENDING TO THE GROUND. EMPLOYEE #2 HAD APPARENTLY BEEN LOADING SOME GLASS INTO ONE OF THE HOISTS AND EMPLOYEE #1 HAD BEEN CALLED TO GROUND LEVEL TO ANSWER A QUESTION. THE TWO EMPLOYEES WERE STANDING IN FRONT OF THE TWO PERSONNEL HOISTS AND HAD NOTICED THAT THE ACCESS DOOR TO THE HOIST WAS OFF THE TRACKS. THE DOOR WAS LEANING INWARD AT THE TOP. THEY BOTH TOOK IT UPON THEMSELVES TO TRY TO PLACE THE DOOR BACK ON THE TRACK. THE HOIST WAS ON ITS WAY DOWN WHILE THEY WERE WORKING ON THE DOOR. OTHER EMPLOYEES ON THE GROUND LEVEL WERE YELLING TO THE HOIST OPERATOR TO STOP, BUT HE COULDN'T HEAR THEM BECAUSE OF THE SURROUNDING NOISE LEVEL. THE HOIST CAME DOWN AND STRUCK THE ACCESS DOOR AT GROUND LEVEL, KNOCKING IT ASIDE. EMPLOYEES #1 AND #2 WERE STRUCK BY THE FLYING DOOR. BOTH WERE INJURED AND TRANSPORTED TO UNIVERSITY HOSPITAL. EMPLOYEE #1 RECEIVED A BROKEN RIGHT TOE AND FOOT. EMPLOYEE #2 RECEIVED AN INJURY TO HIS RIGHT SHIN. HE WAS TREATED AND RELEASED THAT DAY. EMPLOYEE #1 WAS HOSPITALIZED.
