Investigation Summary

Investigation Nr: 14195382
Event: 08/28/1985
EMPLOYEE FRACTURES VERTEBRATE IN FALL FROM SCAFFOLD

On September 3, 1985, Employee #1, a brick or stone mason for Loftus Masonry, was walking on two scaffold grade planks each measuring approximately 10 inches by 2 inches by 16 feet. Supports for the planks spanned approximately 80 inches. The elevation of the scaffold was approximately 13 feet. A crack developed across the width of the plank, approximately 6 feet from the end. The plank failed to support the employee. Employee #1 fell approximately 13 feet sustaining a minor fracture to a vertebrate and several bruised ribs. The plank showed no external damage such as saw marks, knots, or splits. Witnesses did not hear cracking noises. The scaffold sections were tubular welded measuring approximately 80 inches in length, by 80 inches in height, and 63 inches in width. Scaffold components were inspected daily. Employee #1 was a experienced mason of approximately 20 years. He was helping the installation of a brick wall over wood framing at the time of the accident.
