Investigation Summary

Investigation Nr: 14200463
Event: 02/21/1985
FINGERS CRUSHED BY POWER PRESS

A machine operator was machining 0.625-in. diameter C/89 retaining rings on a two-ton Farrel No. 2 mechanical power press. The employee used two foot pedals, one to actuate the press and the other to actuate compressed air. The upper die was 2.25 in. by 1 in. and the lower die was 3 in. by 2.25 in. The distance between the dies was one in. The rings were hand fed into position and actuated between the dies. During a batch process, one of the rings rolled out of the point of actuation. The employee attempted to push the ring back into proper position by using his left hand to flick the ring back to the point of actuation. While attempting this, he accidentally actuated the press and his index and middle left hand fingers were caught between the dies and crushed. He was hospitalized.
