STRUCK BY FALLING WHEEL BALANCER GUARD

EMPLOYEE #1 WAS STANDING BETWEEN THE TIME CLOCK AND THE WHEEL BALANCER WHEN THE RIM AND TIRE GUARD FELL, STRIKING HIM IN THE LOWER BACK. THE GUARD HAD BEEN BROKEN AND WAS WIRED WITH A COAT HANGER IN THE OPEN POSITION. THE COAT HANGER SLIPPED OFF THE NAIL IN THE TIRE RACK, CAUSING THE GUARD TO FALL. THE ACCIDENT HAPPENED AT APPROXIMATELY 7:20 AM. THERE WERE NO WITNESSES. THE EQUIPMENT INVOLVED WAS A FMC COMPUTER WHEEL BALANCER, MODEL 5800, SERIAL NO. 1323. THE EMPLOYEE DOES NOT USE THE BALANCER, AND IT WAS NOT IN USE AT THE TIME. CAUSAL FACTOR: MANAGEMENT DID NOT REMOVE THE MACHINE FROM SERVICE AFTER BEING NOTIFIED OF IT'S DEFECT. THE EMPLOYEE SUSTAINED A STRAIN AND CONTUSIONS OF THE LOWER BACK, NEAR THE BASE OF THE SPINE.
