Investigation Summary
At 5:00 p.m. on March 20, 2012, an employee was assigned to clean the rollers of the Egan calendar machine. The Egan calendar had three rolls. The company's practice was to clean the rollers while they rotated at a speed that was slowed for cleaning purposes. The employee completed the cleaning of the three rolls and was threading a plastic sheet, of finished plastic product through the calendar rolls, in order to prepare the extruder line for startup. The employee threaded the plastic material from under the turning bottom roll (outgoing nip point) and back under the turning middle roll (outgoing nip point). The employee reached over the top of the turning middle roll (ingoing nip point) with his left hand and attempted to grab hold of the plastic material that he was sticking under the middle roller with his right hand. His arm became caught in the ingoing nip point created by the middle and top rollers and he was pulled into the turning rollers. When cleaning the rollers the spaces between the rollers were widened to their maximum width of approximately 3.875 inches. The employee did not pass through the rollers. However, the employee was caught-by the ingoing nip points of the turning Egan calendar rolls. The employee was killed by mechanical asphyxia as a consequence of a crushing chest injury. No one saw the accident and the company did not have any written records of the employees training progress.
