Investigation Summary
At 9:00 a.m. on March 12, 2012, Employee #1 and a coworker were installing air conditioning drops (return and supply ducts) for three new air conditioning units. Both employees were standing on a relatively flat roof of the building approximately 23 feet above the floor surface. While installing the ducts, Employee #1 fell through the 51 inch by 81 inch roof opening onto the concrete floor. Employee #1 was killed in the 23 foot fall.
