HAND AMPUTATED BY BLADE OF A RADIAL ARM SAW

AT APPROXIMATELY 8:30 AM ON AUGUST 3, 1985, EMPLOYEE #1 WAS USING A DEWALT RADIAL ARM SAW TO CUT 2 INCH BY 8 INCH BY 14 FOOT BOARDS. THE SAW, EQUIPPED WITH A 14 INCH RIP BLADE, WAS LOCATED IN THE SAW SHOP AT KAPER'S BUILDING MATERIALS INC., HALLECK AND BEGONIS STREETS, DEMOTTE, INDIANA 46301. EMPLOYEE #1 HAD REMOVED THE LOWER BLADE GUARDS AND THE ANTI-KICKBACK DEVICE FROM THE SAW BEFORE HE BEGAN WORK. HE HAD MADE 3 CUTS OF 4 INCH BY 4 INCH BY 14 FOOT BOARDS BEFORE CUTTING THE 2 INCH X 8 INCH BOARDS TO 10 FOOT LENGTHS. AS HE WAS PLACING A BOARD ON THE SAW TABLE, HIS RIGHT HAND SLIPPED AND CONTACTED THE SAW BLADE. HIS RIGHT HAND WAS AMPUTATED FROM THE THUMB KNUCKLE ACROSS TO THE LITTLE FINGER. ALL PARTS WERE SURGICALLY REATTACHED EXCEPT FOR THE LITTLE FINGER AND INDEX FINGER. THE PRIMARY CAUSE OF THIS ACCIDENT WAS THE USE OF THE SAW WITH THE GUARDS AND ANTI-KICKBACK DEVICE REMOVED. THE EMPLOYEE'S HASTE TO COMPLETE THE JOB ALSO CONTRIBUTED.
