GEAR FROM BALL MILL FELL ON EMPLOYEE, CRUSHING HIM

IN APRIL 19, 1985, AT APPROXIMATELY 8:45 AM, EMPLOYEE #1 WAS HELPING STAND A HALF-MOON BULL GEAR ON ITS SIDE. THE GEAR WAS TAKEN FROM A BALL MILL. THE GEAR WAS LEANED ONTO A 102 AND 1/2 INCH LONG 4 X 4, SO THAT IT COULD BE SANDBLASTED. THE GEAR STOOD 6 FEET TALL, WEIGHING APPROXIMATELY 4,000 POUNDS. THE GEAR FELL OFF THE 4 X 4, STRIKING AND FALLING ON EMPLOYEE #1. EMPLOYEE #1 SUFFERED THE FOLLOWING INJURIES; 3 DEEP LACERATIONS ON HIS RIGHT LEG, A BROKEN RIGHT FEMUR, A COMPOUND FRACTURE OF THE LEFT TIBIA, A FRACTURED LEFT FEMUR AND A FRACTURED PELVIS. THE SCROTUM AND THE ENTIRE GENITAL AREA WAS TORN FROM HIS BODY, ALLOWING THE SMALL INTESTINE TO PROTUDE. THE EMPLOYEE WAS PLACED IN SHOCK PANTS AND GIVEN BLOOD. HE WAS TRANSPORTED TO THE HOSPITAL, WHERE HE WENT INTO SHOCK AND DIED.
