AN EMPLOYEE WAS CONNECTING A SINGLE-PHASE CABLE TO THE DEENERGIZED SIDE OF THE SWITCH FOR ONE PHASE OF A THREE-PHASE SYSTEM. HIS RIGHT HAND APPARENTLY CONTACTED THE SWITCH FOR ANOTHER ENERGIZED PHASE IN THE ADJACENT COMPARTMENT.
