CAUGHT IN A ROTATING SCREW PUMP

ON JUNE 20, 1985, AT APPROXIMATELY 9 PM, EMPLOYEES #1 AND #2 WERE IN THE PROCESS OF CLEANING THE EXCESS GROUT FROM THE BLADES OF A SCREW PUMP. A THIRD EMPLOYEE WAS COMING OUT OF THE PUMP. THE FOREMAN WAS IN THE PROCESS OF GOING DOWN INTO THE SERVICE PUMP WHEN EITHER THE FOREMAN OR THE THIRD EMPLOYEE ACCIDENTLY BRUSHED AGAINST THE PUMP SWITCH AND TURNED IT ON. THE SCREW PUMP WAS AT A 38 DEGREE ANGLE AND WAS 48 FEET LONG AND 6 FEET IN DIAMETER. THE BLADES WERE 5/16 INCH THICK AND THERE WAS 14 INCHES OF CLEARANCE BETWEEN THE BLADE ENDS AND THE INSIDE OF THE PUMP WALL.
