Investigation Summary

Investigation Nr: 14212484
Event: 12/13/1985
EMPLOYEE STRUCK AND KILLED BY END OF WHIPPING STEEL HOSE

EMPLOYEES WERE TRYING TO RUN SURFACE CASING INTO A WELL, BUT BORE CUTTINGS, WHICH HAD SETTLED TO THE BOTTOM, WERE GETTING IN THE WAY. TO ALLEVIATE THIS PROBLEM, MUD WAS CIRCULATED THROUGH THE CASING TO FLUSH OUT THE CUTTINGS. THE MUD WAS CIRCULATED BY CONNECTING A STEEL ROTARY HOSE FROM THE STANDPIPE TO A SWEDGE IN THE TOP OF THE CASING STRING. WHEN THE NEXT-TO-LAST JOINT OF CASING WAS BEING WORKED INTO THE HOLE, A NIPPLE WHICH CONNECTED THE STEEL HOSE TO THE STANDPIPE EXPERIENCED THREAD FAILURE AND PULLED OUT OF THE COLLAR TO WHICH IT WAS ATTACHED. THE HIGH INTERNAL PRESSURE ON THE MUD CAUSED THE THE END OF THE HOSE TO WHIP AROUND ITS ANCHOR AT THE SWEDGE. THE FLYING HOSE-END STRUCK EMPLOYEE #1, KILLING HIM. THIS ACCIDENT COULD HAVE BEEN PREVENTED BY SECURING THE STANDPIPE END THE HOSE WITH A SAFETY CHAIN.
