ELECTRIC SHOCK - GROUND FAULT IN PANELBOARD

A CREW OF EMPLOYEES WAS ERECTING A WATER TOWER. THEY WERE USING A 480-VOLT, 3-PHASE, 210-KILOWATT CATERPILLAR GENERATOR (MODEL NUMBER SR-4) TO SUPPLY POWER TO A JOY AIR COMPRESSOR. THE COMPRESSOR WAS CONNECTED TO A PANELBOARD BY A LABORER AS DIRECTED BY THE FOREMAN. THE LABORER WIRED THE PANELBOARD INCORRECTLY, AND THE WIRING FAULT ENERGIZED THE FRAME OF THE AIR COMPRESSOR. HEAVY RAINS EARLIER IN THE DAY HAD LEFT WATER STANDING UP TO 2 INCHES DEEP AROUND THE COMPRESSOR. THE FOREMAN WALKED BY THE COMPRESSOR TO RETRIEVE SOME EQUIPMENT. WHEN HE STEPPED IN THE WATER, HE WAS ELECTROCUTED.
