EMPLOYEE #1 CLIMBED ONTO A PLATFORM MADE OUT OF 3 BY 12 INCH PLANKS TO TIGHTEN A CLAMP ON THE ROTATING HEAD ON A BLOW-OUT PREVENTER STACK. HE APPARENTLY FELL OFF THE PLATFORM AND STRUCK HIS HEAD ON A METAL OR CONCRETE OBJECT AS HE FELL. HE LANDED IN 11 INCHES OF WATER IN THE CELLAR, AND, BEING UNCONSCIOUS FROM THE BLOW TO HIS HEAD, DROWNED. NO MEANS OF FALL PROTECTION WAS PROVIDED FOR EMPLOYEES WORKING ON THE PLANKS.
