Employee killed when crushed against molding machine

Employee #1 was operating an extruder molding machine that was computerized to make 55-gallon plastic drums. Some guarding existed but it was incomplete, allowing the employee to access the mold area while the machine was operating. As the barrel retriever moved over to receive a drum that was due out of the mold, it struck Employee #1 and crushed him against the mold. He was killed.
