Employee killed when struck by falling headache ball

Employee #1, an experienced crane operator, was performing flagging operations when the hoist line was two-blocked and the 190 lb headache ball was drawn up to the boom nosepoint, creating extreme stress on the wire rope. This caused the ball to fall and strike Employee #1 in the head. He was killed.
