EMPLOYEE DIED DUE TO FALLING CASING PIPE

The employee was standing underneath a suspended 30-ft-long by 7-in diameter casing pipe. The pipe elevating attachment failed, opened, and the pipe fell striking the employee. He sustained injuries to his neck and back and died.
