FINGERS AMPUTATED IN AUGER OF RUBBER EXTRUDER MACHINE

AT APPROXIMATELY 11:50 AM ON JANUARY 27, 1986, EMPLOYEE #1 WAS FEEDING RUBBER INTO THE HOPPER OF A RUBBER EXTRUDING MACHINE. THE RUBBER WAS NOT FEEDING PROPERLY, SO EMPLOYEE #1 REACHED DOWN INTO THE HOPPER (ABOUT 20 INCHES) TO CLEAR THE MATERIAL. HE FAILED TO USE THE PROPER TOOL (AXE HANDLE) WITH WHICH HE HAD BEEN SUPPLIED. THE FINGERS OF HIS LEFT HAND WERE CAUGHT BY THE EXTRUDER'S AUGER, AND ALL 4 FINGERS AND THE THUMB WERE AMPUTATED. THE TOP OF THE HOPPER WAS ABOUT 66 INCHES ABOVE FLOOR LEVEL AND WAS LOCATED BETWEEN STEEL COLUMBS. A STEP HAD BEEN INSTALLED TO HELP SHORT EMPLOYEES ON THIS JOB. THE STEP ENABLED EMPLOYEE #1, WHO IS TALL, REACH FAR DOWN INTO THE HOPPER.
