Employee killed in fall down elevator shaft

Employee #1 took a freight elevator to the ninth floor to bring a malfunctioning passenger elevator down to the lobby. After talking with a guard and the building manager, Employee #1 walked outside and smoked a cigarette before returning to the lobby. When he opened the elevator door and stepped in, he fell 60 feet down the elevator shaft. Employee #1 was killed.
