EMPLOYEE #1 WAS RIDING ON TOP OF THE SAFETY CAGE ("MONKEY CAGE), ON THE CARRIAGE OF THE STACKER CRANE. THE CRANE IS A MAJOR COMPONENT OF THE AUTOMATIC STORAGE AND RETREIVAL SYSTEM. HE WAS CRUSHED BETWEEN THE SAFETY CAGE AND THE STEEL CROSS MEMBER WHICH CONNECTS THE DUAL MASTS OF THE STACKER CRANE, 90 FEET ABOVE FLOOR LEVEL.
