CONSTRUCTION EMPLOYEE KILLED IN FALL THROUGH UNGUARDED HOLE

AT APPROXIMATELY 8 AM ON MARCH 27, 1985, EMPLOYEE #1 WAS NOTIFIED BY TWO OTHER EMPLOYEES THAT A 2 TON CAPACITY HOIST APPARENTLY HAD NO POWER. EMPLOYEE #1 ASCENDED THE STAIRS TO THE PLATFORM, APPROXIMATLEY 98 FEET ABOVE FLOOR LEVEL. THE HOIST WAS AT THE OTHER END OF A 122 FOOT LONG PLATFORM. HE STARTED WALKING TOWARD THE HOIST AND WAS MET BY THE 2 OTHER EMPLOYEES. THE HOIST WAS MOUNTED ABOUT 8 FEET ABOVE THE PLATFORM. THE THREE MEN STARTED WALKING TOWARD THE HOIST. AS THEY NEARED THE HOIST THE THREE WERE DISCUSSING THE PROBLEM AND LOOKING AT RELATED ELECTRICAL SWITCHES ON AND AROUND THE HOIST. EMPLOYEE #1 STEPPED FORWARD, UNDERNEATH THE HOIST AND INTO AN UNGUARDED OPENING IN THE FLOOR UNDERNEATH THE HOIST. HE WAS KILLED IN THE 98 FOOT FALL TO THE CONCRETE FLOOR. TWO OTHER EMPLOYEES HAD JUST OPENED THE FLOOR HOLE IN PREPARATION FOR THE LOWERING OF THE HOIST BLOCK THROUGH THE OPENING.
