TWO MEN FELL (HEIGHT UNKNOWN) WHILE WORKING FROM CABLE SUPPORTED SCAFFOLDING INSIDE A WATER STORAGE TANK UNDER CONSTRUCTION. THE CABLES WERE ATTACHED TO STEEL LUGS WELDED TO TANK COMPONENTS. ONE LUG PULLED LOOSE ALLOWING SCAFFOLDING TO FALL. BOTH MEN FELL TO TANK BOTTOM.
