EMPLOYEE #1 WAS THE OPERATOR OF A TRUCK MOUNTED SOIL TEST DRILLING MACHINE. HIS COAT SLEEVE BECAME CAUGHT BY THE MACHINE'S ROTATING SHAFT. THE SHAFT PROPELLED HIM AGAINST THE MACHINE STRUCTURE, CAUSING FATAL INJURIES.
