Investigation Summary
AT APPROXIMATELY 7 AM ON OCTOBER 24, 1986, EMPLOYEE #1 AND 3 OTHER EMPLOYEES ENTERED A CONCRETE TANK TO GROUT PUMP PADS LOCATED AT THE BOTTOM OF THE TANK. TO GAIN ACCESS TO THE TANK THE 4 EMPLOYEES DESCENDED THE 13 1/2 FOOT LONG TOP PORTION OF A METAL EXTENSION LADDER. THE LADDER WAS NOT BLOCKED, OR OTHERWISE SECURED, TO PREVENT ITS DISPLACEMENT. IT WAS ALSO SEVERELY BENT. THE TANK WAS 12 FEET DEEP. THE FOUR EMPLOYEES DESCENDED THE LADDER WITHOUT INCIDENT. AS THEY NEARED COMPLETION OF THE WORK, EMPLOYEE #1 AND ANOTHER EMPLOYEE DECIDED TO LEAVE THE TANK. EMPLOYEE #1 BEGAN CLIMBING THE LADDER BEFORE HIS CO-WORKER COULD HOLD THE LADDER FOR HIM. HE HAD CLIMBED 4 OR 5 FEET WHEN THE LADDER "KICKED OUT" ON HIM. AS A RESULT, HE FELL, STRIKING HIS HEAD ON THE CONCRETE FLOOR. THE EMPLOYEES HAD BEEN INSTRUCTED IN THE PAST TO ALWAYS SECURE LADDERS TO PREVENT DISPLACEMENT. HOWEVER, THIS WORK RULE WAS NOT ENFORCED DUE TO THE LACK OF A DISCIPLINARY ACTION PLAN. EMPLOYEE #1 SUSTAINED SEVERE HEAD INJURIES, RESULTING IN DAMAGE TO THE LEFT SIDE OF HIS BRAIN.
