AT ABOUT 12 NOON ON MARCH 21, 1987, EMPLOYEE #1 WAS STRINGING WELDING CABLE ON THE ROOF OF WAREHOUSE #4, 22 1/4 FEET ABOVE AN ADJACENT CONCRETE FLOOR. THE 85 POUNDS OF WELDING CABLE HE HAD SLUNG OVER HIS LEFT SHOULDER CAUSED HIM TO LOSE HIS BALANCE AND FALL OFF THE ROOF TO THE CONCRETE FLOOR. HE WAS HOSPITALIZED WITH HEAD INJURIES.
