CONSTRUCTION EMPLOYEE DROWNED WHEN HE FELL FROM PLATFORM

ON SEPTEMBER 29, 1987 EMPLOYEE #1 AND ANOTHER EMPLOYEEE WERE WORKING FROM A 28 FOOT LONG, 10 FEET, 10 1/2 INCHES WIDE SUSPENDED PLATFORM. AS THEY WERE LOWERING THE PLATFORM ONTO A PAIR OF 5/8 INCH HORIZONTAL ROPES USED TO TRAVEL FROM FLOOR BEAM TO FLOOR BEAM (TO INSTALL REINFORCING PLATES), ONE CORNER OF THE PLATFORM SOMEHOW BECAME RELEASED. EMPLOYEE #1 ABOUT 86 FEET INTO THE SUSQUEHANNA RIVER, BOUNCING OFF PARTS OF THE STRUCTURE ON THE WAY DOWN. THE EMPLOYEE APPARENTLY DROWNED. NO FALL PROTECTION DEVICE (IE., SAFETY NET) OR PPE (IE., FLOTATION VEST) WAS PROVIDED.
