Employee killed when crushed between truck and storage

Four employees were standing in an area at the rear of a 40-ft-long storage trailer. The employees were waiting for a 1983 International, two-axle van-type truck with a 24-ft-long body. The truck was backing up to the storage trailer so that it could be loaded with cartons of Class C fireworks. An employee was standing on the driver's side of the truck directing the truck driver. Another employee was standing on the passenger side of the truck. Employee #1 and a coworker were standing directly behind the truck. The truck was moving backwards, on an angle, and traveled further than intended. Employee #1 was caught between the rear of the truck and the left side of the storage trailer which had its doors open. Employee #1 was killed when the truck crushed his chest. The employees had not received training on the recognition and avoidance of hazards associated with staying clear of trucks backing up. The employees had not received training in safely directing the truck driver.
