EMPLOYEE KILLED WHEN CAUGHT BETWEEN FORKLIFT HANDLE AND WALL

EMPLOYEE #1 REPORTED TO WORK ON THE DAY OF THE ACCIDENT AT HER NORMAL TIME OF 4 PM. OPERATIONS THROUGHOUT THE PLANT WERE RUNNING BEHIND SCHEDULE, AND AT 4:30 PM SHE HAD TROUBLE DRAINING THE FLUSH WATER OUT OF THE #6 FILTER FEED. HER FOREMAN TRIED TO ASSIST HER. AT 5 PM HE TOLD HER HE WOULD TAKE CARE OF THE FILTER AND THAT SHE SHOULD GO AND START CATCHING UP ON HER REGULAR WORK. FROM 8 PM UNTIL 8:30 SHE WAS SEEN EATING HER EVENING MEAL. AT 8:30 SHE RETURNED TO THE WEIGH ROOM TO CONTINUE HER DUTIES. AT ABOUT 9:05 PM, ANOTHER EMPLOYEE WALKED BY THE WEIGH ROOM AND NOTICED THE BATTERY POWERED YALE FORK TRUCK (MOD #255071EE, SER #236880) SITTING IN AN UNUSUAL POSITION BENEATH THE PALLET RACKS. HE WALKED OVER AND NOTICED THAT THE HANDLE OF THE WALK-ALONG FORKLIFT HAD PINNED EMPLOYEE #1 UNDER THE RACK AND AGAINST THE WALL. THE HANDLE WAS IN HER CHEST AREA, HER BODY SLUMPED OVER THE GUARD AND HER FEET OFF THE CONCRETE FLOOR. CPR WAS STARTED. THE EMERGENCY CREWS NOTICED THE GUARD OVER THE CONTROL SWITCH WAS BROKEN. HER RIGHT HAND WAS CAUGHT BETWEEN THE GUARD AND SWITCH BECAUSE OF HER BODY WEIGHT ON THE GUARD. THE EMPLOYEE WAS KILLED.
