ELECTRIC SHOCK - DIRECT CONTACT WITH OVERHEAD LINE

AN EMPLOYEE WAS CHECKING A NOISE PROBLEM ON A TELEPHONE LINE. AFTER ISOLATING THE TROUBLE TO A UTILITY POLE WHICH WAS EQUIPPED WITH TWO TRANSFORMERS, THE EMPLOYEE BEGAN TO CHECK THE LINE FROM THE FIBERGLASS BUCKET OF AN AERIAL LIFT. HE MOVED THE BUCKET UP TO THE TRANSFORMER LOCATION (ABOUT 2 FEET ABOVE THE TELEPHONE LINE) AND TRIED TO TIGHTEN THE TOP WIRE ON ONE OF THE TRANSFORMERS. THE EMPLOYEE MADE SIMULTANEOUS CONTACT WITH BOTH LINES OR WITH ONE LINE AND GROUND, AND CURRENT TRAVELED FROM HAND TO HAND. HE WAS ELECTROCUTED.
