Employee crushed when pulled into a revolving wire spool

On December 12, 1989, an Employee was working around lubrication area of die boxes while machine was in operation. Wire was being drawn through the die boxes when the wire snagged an employee and pulled him into the revolving wire spool. The employee suffered fatal crushing injuries to his neck and chest area resulting in his death.
