Investigation Summary
Employee #1 was laying storm pipe in a 100 ft long by 9 ft deep trench when the side wall next to the road gave way and pinned him in an upright position. It took 6 hours to extract him. The employee suffered a ruptured spleen and possible fractures of the pelvis and ankle. No trench box or shoring was used. The workers attempted to slope the trench, but it was inadequate for type C soil and conditions.
