Investigation Summary
Employees #1 and #2 and their foreman were ascending the side of a 5-story office building on a Werner model #2624, 500 lb capacity, 2-point suspension scaffold to commence painting. The power to the two hoist motors went dead with the scaffold approximately 22 ft above the asphalt parking lot. The foreman left the scaffold to restore power. Employees #1 and #2 remained on the scaffold to pull up power cords to plug into an alternate power source. Employees #1 and #2 untied themselves from their lifelines and the foreman tied the power cord to one lifeline. Employees #1 and #2 pulled the cord up to the scaffold. The foreman went to the third floor to plug in that power cord. Employees #1 and #2 handed the cord to the foreman through a window and the foreman plugged it into an outlet. The roof anchorage for the scaffold gave way because the brick masonry parapet collapsed and one end of the scaffold fell to the ground along with Employees #1 and #2. Employee #1 was killed and Employee #2 sustained injuries to his head and right leg and torso. The parapet clamp on the west end of the scaffold was not secured to the structure, and Employees #1 and #2 were not secured to lifelines.
