Investigation Summary
Employee #1 was standing beside a loaded truck trimming a branch from a tree when the loader operator attempted to move a tree that was not properly positioned on the truck. The tree, approximately 7 to 8 in. in diameter and 35 to 40 ft long, fell off the truck and struck Employee #1 on the head and neck, killing him.
