Investigation Summary
Employee #1, the operator of a forklift, was removing a pallet of four coils of steel from the top of a storage rack that was 8 ft from the floor. When the load was lifted, the forklift tilted forward and the employee was thrown into the air. He fell to the floor and landed on his back. The forklift then fell on the employee's left arm and the left side of his chest. He was hospitalized.
