Employee's arm amputated after getting caught in chain drive
On August 7, 1990, Employee #1, age 59, became caught in an unguarded chain drive. His hand and arm were trapped in the machine frame opening while the chain was moving. His lower right arm was later amputated. Proper guarding of the chain drive could have prevented the accident.
