Investigation Summary
A fork-lift operator left the operator's seat and walked to the front of the fork-lift. The load of three large ductile iron pipes on the raised forks was unstable and the front pipe fell on the employee. The employee may have brushed the end of the pipe dislodging it. The 865 pound pipe crushed the employee's left shoulder and skull which resulted in massive head trauma killing the employee.
