Employee killed when head is crushed in pinch point

At approximately 7:10 a.m. on April 19, 1991, Employee #1 climbed beneath a machine to retrieve scrap material. Her head entered the pinch point between the support bar on the machine and a descending apparatus, and was crushed. Employee #1 was killed.
